PreClinical years journey (MBBS)

Basically my writing for this one is all about the preclinical years of Bachelor of Medicine and Surgery (MBBS) which is first year and second year (generally in Malaysia). And to be more specific, ini adalah post berkenaan sedikit perkongsian pengajian saya sepanjang tahun 1 dan tahun 2 di Widad University College (WUC).

During my first year, we studied according to few modules. Please do not ask me the title of the modules, I cannot remember. But overall, first year consists of few subjects;
1. Anatomy
2. Physiology
3. Biochemistry
4. Pathology
5. Microbiology
6. Immunology
7. Pharmacology
8. Genetics
9. Nutrition
10. Public Health

Some are combined in one module such as physiology and biochemistry in Functions and Metabolisms of Cells and Tissues module.

First year was the year where we learned everything generally. Anatomy, which I will never forget when we have to know everything from head to toe, from muscle to its origin, insertion, action, innervation, blood vessels, all organs, their locations, parts, relations, innervations and blood vessels. Upper limbs, lower limbs, head and neck. And all other things in a very very short period.

Starting second year, we studied based on systems. 4 systems in 1 semester.

Semester 3

1. CNS (central nervous system) and LNS (locomotor nervous system) - 13 credit hours
2. CVS (cardiovascular system) - 5 credit hours
3. Respiratory system - 5 credit hours
4. Renal sytem - 5 credit hours

Semester 4
1. GIT (gastrointestinal tract) - 10 credit hours
2. Endocrine - 4 credit hours
3. Reproductive system - 6 credit hours
4. Haematology - 3 credit hours

Each system consists of 2 quizzes and 2 test roughly. And also Problem-Based Learning (PBL).

For pbl, we were divided into groups. Each group with 7 members. For 1 pbl, each consists of two sessions. Session 1 (open case) and session 2 (close case). Pbl will be held on friday for each session which means 1 case completed within 2 weeks. 

Setiap session akan ada rotation lecturer yang guide for each group. Masa open case ni lah kita akan dapat case dan bincang on what to present for next session. Ada lah format dia. Kadang-kadang kena ikut lecturer, ada lecturer nak cara macam ni, ada lecturer nak cara lain. Sebelum close case, kena bahagi task dan prepare lah sebaiknya. Walaupun pemarkahan adalah secara individual, tapi janganlah fikir macam kalau aku tak buat part aku, teammates lain tak affected pun. NOOOO!! The others will surely be affected sebab bila seorang tak perform, akan spoil mood Dr. Nanti Dr kata tak ada kerjasama dan kita tak discuss wholly pun pasal case tu.

For carry marks, collected from all the quizzes, test, some presentations needed, lab reports, tutorial and pbl. And each semester mestilah ada End of Semester Examination (EOS) ataupun final exam.

Untuk EOS, akan ada multiple choice question (MCQ) plus single best answer (SBA), modiffied essay question (MEQ) dan objective structural practical examination (OSPE).

MCQ tu macam soalan objektif tapi jawab true/false for each option (A,B,C,D,E). Biasanya one paper consists of 25 MCQs (kalau tak silap). So darablah 25 dengan 5 so total soalan kena jawab 125 questions. Dan MCQ ni ada negative marking which is kalau salah jawab satu soalan akan minus 0.5 marks so kenalah bijak buat keputusan. Kalau tak sure tu better kosongkan je daripada buang markah free2 macam tu je. Aku faham perasaan students yang dari sekolah lagi tak biasa kosongkan ruang jawapan, rasa tak complete kan sebab aku pun macam tu. Tapi dah degree ni janganlah suka suka bazir markah. Be wise ok. SBA pula ada 10 questions. SBA ni macam soalan objektif biasa cumanya pilihan jawapan dia semua betul, kita kena pilih the best one. Tricky sebenarnya SBA ni. MCQ dan SBA are in one paper.

Next, MEQ. Lebih kurang structured question. Untuk year 1, sebab still belajar generally, so soalan dia bukanlah in case form. Ada 5/6 soalan (tak ingat) berserta anak-anaknya. Untuk year 2, soalan dia based on cases. Ada 6 cases ikut system. Dan macam biasa ada anak-anaknya. Dalam satu case tu, dia akan bagi satu history, examination and investigation (kalau ada). Ada case yang bagi brief situation, ada yang bagi sikit je info. Kena pandai-pandai detect system apa dan diagnosisnya. Soalan-soalan dia pula ikut subjek; anatomy, physiology, pathology etc. Lagi banyak soalan lagi bagus sebenarnya sebab pemarkahan for each question akan lebih kecil. So takdelah banyak hilang markah kalau tak dapat jawab. Yang penting, DON'T WRITE RUBBISH. Jangan tunjuk sangat kita tak tahu dan tak study. Dr marah sangat. Aku pernah kena marah sebab ni huhu. Jujur je ni sebab masa tu masa semester 1 memang tak study untuk satu quiz tu je sebab sakit teruk.

OSPE ni beza sikit. Ni kira benda barulah. Ada 25 soalan yang perlu dijawab at 25 different stations. Pembahagian stations adalah mengikut subjek. Contoh, 5 stations untuk pathology, 5 stations untuk anatomy etc. Soalan-soalan dia macam menarik sikit. Kalau anatomy tu, Dr mungkin akan letak mana-mana bone dan tanya nerve apa yang supply muscle kat bone tu. Dan banyak lagilah contoh soalan lain. Microbiology paling tak best sangat sebab most of the questions kena tengok slide under microscope. Faham tak benda tu semua nampak lebih kurang je hahaha. Physiology biasa Dr bagi soalan dalam bentuk chart then fill in the blank. So kena alert semua diagram, chart, graph dalam slide Dr. Yang lain-lain tu pun lebih kuranglah, panjang pula nak tulis satu-satu. Each station perlu dijawab dalam masa 3 minit sahaja. 3 minit tu sangatlah panjang untuk soalan yang mudah dan kita tahu serta ingat jawapan dia but not for questions yang confusing dan kita tak ada idea langsung jawapan dia.

Sebelum naik tahun 3 which dah start clinical year, akan ada Professional Exam 1. Kena pass kalau nak naik clinical year, kalau tak pass kena repeat exam, kalau tak pass lagi sekali kena repeat semester. From this examination, akan ada Viva (oral examination gitu). Viva ni untuk students yang markah dia boleh naik ke distinction ataupun borderline (almost fail). Examiner viva adalah external examiner, bukan lecturer kita sendiri.

Pointer seeloknya jaga from the beginning sebab makin lama makin susah. Kalau boleh target deanlist untuk awal-awal ni. Lagi bagus kalau sampai habis semester 10. Ada je senior yang maintain pointer above 3.5 sampai grad dan sampai current semester which dah tahun 4.
